This shot was taken from the road at 41.541653, -112.091909 with the camera facing South towards the barn located at 41.539250, -112.092142.

The title card shot was taken from the ramp to I-84 at 41.6347371, -112.1024035 with the camera facing towards the area at 6680 N 3600 W. This view has changed quite a bit since 1988 as some of the original houses and barns have been demolished and rebuilt, but the large metal shed is still there.

After an extensive search I wasn't able to find this location so the building has most likely been demolished. If any residents of Corinne, Bear River City, Honeyville or the surrounding area know where this is (or was) then please let me know.
This shot was taken from somewhere in the Bear River City area with the camera facing West towards Little Mountain. Identifying the exact location is too difficult but I think it was somewhere on 5200 W or close-by. I believe the photo below was taken from the area at 41.625748, -112.135104.
The next three shots in the montage (the barbed wire, scarecrow with the axe, and the pitchfork) were all filmed around the same barn, which is located at 41.632313, -112.130134 in Bear River City.

The building that doubled as Ridgemont Federal Sanitarium was constructed in 1932 and originally served as a Veterans Administration Hospital until its closure in 1962. The building was then purchased by the LDS Church and functioned as an annex to the Primary Children's Hospital located down the road on 12th Avenue, until its closure in 1990. The building then remained vacant until 2005 when work began to convert the old hospital into The Meridien, a luxury development of condominiums.

Two locations were combined to form the interior of Ridgemont Sanitarium. The first location was the Utah State Prison, where the Smith's Grove attendants arrival at the guard station was filmed, along with the scenes in the depths of the sanitarium where Michael is been held. According to Art Director Roger Crandall in an old interview, some of the filming actually took place in the prison's old death row area.
The second location was the George E. Wahlen VA Medical Center, where the Smith's Grove attendants journey from the guard station to the elevator was filmed. Apparently there was footage of the attendants looking into the cells but this was cut from the film and has never surfaced. The other scene filmed at this location was Loomis' journey down the long corridor to see Hoffman and the subsequent scene between them in his office. Although shot at the same location this corridor does not seem to match those from the climax of the Halloween 6 Producer's Cut.
The Carruthers' house makes its first appearance in the Haddonfield establishing shot. The interior was actually used in the film and Jamie has the bedroom at the front of the house, but this would change for Halloween 5.
The house has since been renovated and remodelled but it's still recognisable. The biggest change compared to the film is the kitchen and dining area, which is now open plan. The following photos were taken in 2019 when the house was put up for sale.
This area has since become overgrown which makes it more difficult to see the bridge from the same angles as the film. Although it's not shown on the comparison photo the barrier has recently been extended so it wouldn't now be possible to pull in from the same point.

This location (which dates back to the 1940's) has changed considerably since 1988 as it's no longer a service station and the diner has been heavily renovated and remodelled. The interior wall with the Abraham Lincoln photos has been removed so that the diner could be extended into the area behind it, and the doorway into the kitchen where Loomis sees The Shape standing has since been converted into a hatch.
After being taunted Jamie runs out of the school's South entrance, which still exists but not in the same form as this part of the school was demolished and rebuilt in 2010. She then walks alongside a distinctive wall before being picked up by Rachel and Lindsey at the intersection of East 700 South and Douglas Street.
The school then reappears near the end of the film with Earl and his crew arriving at the school's West entrance, which no longer leads outside but now provides access to the newer part of the school that was built in 2010.

Lindsey makes the turn from East 800 South onto Elizabeth Street, where the drive continues until the end of the scene.
Vincent Drug was a real store that operated at this location from the 1940's until its closure in 2003. The store has remained vacant ever since and the large Vincent Drug sign was taken down in 2017 as part of an estate sale. The sign then made its way into an antique store and was purchased by Graystone Mortgage, who hung it in the lounge of their corporate HQ.
Trivia: This location also makes an appearance in The Sandlot (1993) and The Stand (1994)
The Vincent Drug interiors were filmed at the Millcreek Pharmacy in Murray, which has since closed after 60 years in operation. The building has now been renovated and remodelled and as of 2022 is home to a beauty salon. The counter that Brady works behind was located where the black storage units now are, whilst to the left and just out of shot is the window where Kelly Meeker was working.

Jamie and Rachel start at 282 North Canyon Road before moving next door to 278 North Canyon Road. From there they move to the Meeker house, with The Shape watching on from the garage at the corner of Spencer Court and Canyon Side Road.

This house (known as the Ellis Mansion) was vacant at the time of filming, making it an ideal location as quite a bit of the action takes place here. Although Rachel and Jamie's stunt doubles did do some work on the roof, most of those scenes were filmed on a false roof that was built in a field somewhere in the Mountain Dell Canyon area (where the Little Dell Reservoir now is).

As far as I can tell Earl's bar was known as the Lead Mine Saloon at the time of filming but then became the Ore House Saloon in the 1990's. The building sustained fire damage in 2009 and was subsequently restored, only to close and be demolished two years later in 2011.
Trivia: This location also makes an appearance in Zack and Reba (1998).
Fan favourite Bucky appears next in a memorable scene at the power station, which is actually the substation in Midvale. Haddonfield is plunged into darkness when Bucky is thrown onto the circuit breakers and we see two houses losing power. The first house is located at 521 3rd Avenue, Salt Lake City (two down from the Carruthers house), followed by the house next door at 515 3rd Avenue (The Cornish house from Halloween 5).

When Rachel realises that she has lost Jamie she is standing directly in front of the house at 232 Canyon Side Road. She then moves to the far end of this road where The Shape watches her through the window of the garage at the corner of Spencer Court. Rachel turns around and The Shape quickly moves away from the corner of the same garage (where he was also watching Jamie and Rachel trick-or-treating at the Meeker house earlier).
When Jamie searches for Rachel we see her walking alongside a now non-existent wall in front of 236 North Canyon Road (on E 200 N), before the camera focuses in on a pumpkin on the porch. She then moves onto North Canyon Road and peers around the corner of the same house, towards the area where the trick-or-treating kids were picked up earlier.

Rachel then continues up the small path at the far end of Canyon Side Road, passing a garage before seeing The Shape in the distance. She then runs down the driveway and climbs the metal fence between 248 and 252 North Canyon Road. We then see her climb another fence between 272 and 278 North Canyon Road.

Rachel finds Jamie in front of 248 North Canyon Road and both are then picked up by Sheriff Meeker (the building in the background is Ottinger Hall at 233 North Canyon Road). The first pseudo Michael can then be seen standing between 278 and 282 North Canyon Road (in reality Loomis wouldn't have seen him). The second pseudo Michael then emerges between 238 and 244 North Canyon Road and finally the third pseudo Michael appears from the grass area beside Ottinger Hall. Meeker then pulls away (heading South) and the real Michael can be seen standing in front of Ottinger Hall, but he's facing North in the opposite direction.

The building that served as the Haddonfield Police Department was the former Midvale City Hall (originally the Salt Lake County Library). Interiors were also filmed at this location, and whilst it's difficult to confirm I believe this took place in the Council Chamber room to the right of the main entrance. In 2016 the building was unfortunately demolished following the relocation of the city offices.

I strongly suspect the 'driving out of Haddonfield' scenes were filmed on a stretch of UT-65 in Mountain Dell Canyon that no longer exists, having been lost to the construction of Little Dell Reservoir between 1988 and 1993. At the time of filming a new road had already been constructed around the site of the reservoir (as it is now), leaving the original road unused and available to the production. It looks like several parts of the road were used but most of the scenes were actually filmed on the same bend (also seen when the ambulance is on route to Ridgemont). It's very difficult to confirm but I believe this bend was located in the area at the base of the dam (see the map below).
The subsequent shootout at the abandoned mine shaft (actually a hole dug by the crew) was filmed in a wooded area just off the same road, in the middle of where the Little Dell Reservoir now is (see the map below for the general area).
Thanks to Art Director Roger Crandall for revealing the location of the mine shaft. I searched extensively but I never would have found it without the information he provided.
