The film opens with The Shape escaping from the mine shaft in the Storm Mountain Picnic Area of Big Cottonwood Canyon (a change from Mountain Dell Canyon in Halloween 4). The Shape crawls from the rocks and plunges into the Big Cottonwood Creek in the area at 40.624432, -111.744482, before travelling downstream to the hermit shack, which was actually built upstream in the area at 40.624970, -111.744565.

The Haddonfield Children's Clinic building is actually the former Telluride Institute College for the study of electrical engineering, which is located at 40.315889, -111.655250 on the Olmsted Hydroelectric Plant campus.

The Carruthers' house returns but in a change from Halloween 4 Rachel now has Jamie's bedroom at the front of the house, with Jamie moving into the bedroom next door, although the film makes it appear that her bedroom is located at the end of the hall.
The house has since been renovated and remodelled but it's still recognisable. The biggest change compared to the film is the kitchen and dining area, which is now open plan. The following photos were taken in 2019 when the house was put up for sale.

The Man in Black arrives into town in front of Vincent Drug, which is also the drug store where Brady works in Halloween 4.
Vincent Drug was a real store that operated at this location from the 1940's until its closure in 2003. The store has remained vacant ever since and the large Vincent Drug sign was taken down in 2017 as part of an estate sale. The sign then made its way into an antique store and was purchased by Graystone Mortgage, who hung it in the lounge of their corporate HQ.
Trivia: This location also makes an appearance in The Sandlot (1993) and The Stand (1994)
One of the most controversial aspects of the film is the Victorian style Myers house, which looks nothing like the original version. As well as the exteriors the interiors were also filmed here, with a few exceptions. Both the attic area and the interior of the laundry shoot were filmed on an unknown soundstage in Salt Lake City. Aside from that, the basement area with the bottom of the laundry shoot was filmed in the basement of the Children's Clinic building in Orem.
In the time since 1989 the house has received a new multicolour paint job and its interior has been completely renovated, but the areas used in the film are still recognisable (a distinctive piano that appears in one scene is still there). The interior photos below were taken in 2020 when the house was put up for sale.

The Shape first watches Tina and Sammy leaving the Open Classroom School at 134 D Street before switching his attention to Mikey outside of the Quick Food Mart, which is now a Chinese restaurant called Foodie and Sweetie D Market. Mikey then pulls around the back of the store and is subsequently killed.

This gas station was demolished between 1993 and 1997 and has since been replaced with an office building.
Jamie runs into Billy at the North West corner of East 1st Avenue and R Street. We later see them running north on R Street from exactly the same spot, before hiding behind the second tree along to avoid the passing police car.

Probably the most iconic location from the film is the Tower farm, which I was proud to have been to first to find. The house, barn, fields and wooded areas were all heavily used for filming, but doesn't appear to have changed at all since 1989. However, false trees were brought in for The Shape to drive through in pursuit of Jamie.

The Haddonfield Police Department building was the Layton City Police Station, which was demolished between 1990 and 1991 after a new Justice Center had been constructed just south of where the original building stood (now a parking lot). A small part of the original building does still exist (home to the Davis Arts Council) but this was not the police wing.
